Overview for Appropriate Use of Palliative in the Skilled Nursing Facility: Alliant Quality, in partnership with Skilled Nursing Facilities (SNF) and Palliative Care Organizations, provided education and learning sessions for SNF leaders to better understand palliative care and its beneficial role for the residents. These sessions were successfully pilot tested over the past two years in central North Carolina and in southeast Georgia. Alliant is now providing a toolkit for other SNFs to get the same results of reduced acute care transfers.The introductory video is a testimony to the benefits of providing palliative care in SNFs from nursing home leaders, residents and family members.Presented by:Jennifer
